気管支ぜんそく

アトピー体質の子がなりやすい、アレルギー性の病気。
ヒューヒュー、ゼーゼーと呼吸困難の発作を繰り返します。
発作の程度は、日常生活に支障のない軽いものからチアノーゼを起こすような重いものまで、さまざま。
1才未満で「気管支ぜんそく」と診断されることはまずありません。

どんな病気？

一般に小児ゼンソクと呼ばれているもので、アレルギー反応によって呼吸困難の発作を繰り返す病気です。発作が起きると、ヒューヒュー、ゼーゼーと呼吸が苦しそうになって、せき込んだりします。小さな発作の場合はそれほど苦しくもなく、ふつうの生活を送れます。

発作の引きがね、程度は人によって異なります
発作は、空気の通り道である気道が、あらゆるものに対して敏感に反応するため起こります。発作が起きているときは、気道に一時的な収縮が起こり、気管の壁がむくみ、たんの分泌がふえ、気道が細くなっています。そのため呼吸がしにくくなり、ヒューヒュー、ゼーゼーという喘鳴が聞こえるのです。
　発作は、天候の変化や精神的なストレスが引きがねになったり、ハウスダスト、花粉、食べ物などが原因の場合もあります。かぜや気管支炎などの病気への感染が、最初の発作を引き起こすきっかけとなることも。
　発作の程度は人によってまちまち。小さな発作なら軽い喘鳴はあってもそれほどの呼吸困難はなく、ふつうに日常生活を送ることができます。中ぐらいの発作になると、喘鳴もひどく、呼吸回数が多くなったり、肩で息をするなどのほか、不機嫌になる、元気に遊べない、眠りが浅い、食欲がない、などの様子も見られるでしょう。大きな発作になると、さらに呼吸が苦しそうになり、横になって眠ることもできなくなり、チアノーゼといって唇が紫色になってしまうこともあります。チアノーゼを起こしたり、けいれんを起こして意識が薄れたりしたら、たとえ夜中でもすぐに医師の診察を受けてください。

1～2才では診断がむずかしい病気

気管支ぜんそくという診断がつくのは1才を過ぎてからがほとんど。1才未満の赤ちゃんでは「気管支ぜんそく」と診断されることはあまりなく、1才未満でゼロゼロしているのはウイルス性気管支炎などで、ゼンソク様気管支炎と呼ばれるものが大多数です。ゼンソク様気管支炎の場合は、4～5才ぐらいまでに自然に治ってしまいます。
　1～2才の子どもの場合、気管支ぜんそくであるかどうかの診断はなかなかむずかしいもの。生まれてからの経過、ゼロゼロ音の聴診、アトピー性皮膚炎にかかっているかなど、その子がアレルギー体質かどうかをいろいろな面から調べてようやく診断がつきます。

遺伝的要素の多い病気

両親のどちらかが子どものころ気管支ぜんそくだったり、アトピー性皮膚炎や花粉症などのアレルギーの病気を持っている場合、その子どもにもアレルギー体質が遺伝して、気管支ぜんそくが見られるケースが多いようです。
　
気管支ぜんそくが疑われるのは次のような場合です。

1）0才まではゼロゼロしなかったのに、1才過ぎてから息を吐きだすときにゼイゼイするようになってきた。
2）ゼロゼロするときに、熱は出ないことが多い。
3）アトピー性皮膚炎と診断されたり、湿疹ができやすい。
4）両親やきょうだいにアトピー性皮膚炎や気管支ぜんそくの人がいる。

最初の発作は2～5才までに起こることが多く、小学校に入学するまでに治ってしまう子どももいます。でも、思春期ごろまでは発作を持ち越して、その後自然に症状が改善したり、治ったりするというケースがほとんど。一度こうした発作を起こすと気道は過敏になり、いろいろな刺激がきっかけになって発作を繰り返しやすくなります。環境をととのえるなどして、なるべく発作を起こさないよう注意してあげてください。

ケアーは？

ゼンソクの発作が見られたら、その原因を突き止めるために必要な検査を病院で受けて、家庭での過ごし方などのアドバイスを受けます。発作を起こしたときの状況などは、あらかじめメモしておいて、正確に医師に伝えるようにします。
　発作が起こったら、とりあえず発作を抑える対症療法をします。治療で使われるのは、気道の炎症を取る薬と狭くなった気管支を広げる薬（気管支拡張剤）の2種類です。炎症を抑えるにはステロイド剤が効果的で成人ではよく使われますが、長く使うと副反応が出るので、赤ちゃんでは重症の場合のみ使われます。

軽い発作なら、呼吸がラクになるケアを

発作を起こしたら、まず抱き起こして、少し前かがみにすわらせたり、たて抱きにしてあげるとラクになります。発作が軽いときは、衣服をゆるめて、たんが切れやすくなるように水分を少量ずつたっぷり与えます。そしてゆっくり大きな呼吸ができるよう、リズムをとってあげると効果的。それでも発作がおさまらない場合は、収縮して狭くなった気道を広げる気管支拡張剤や吸入薬を、医師の指示に従って使います。　発作は夜中にひどくなることが多いので、ママは必ず赤ちゃんの隣に寝るようにして、発作が起きたときはすぐケアできるようにしましょう。発作がおさまらず、チアノーゼがあらわれたりしたら、すぐ受診して。
　発作のつらい状態を見ていると、ついつい、過保護になってしまいがちですが、発作がないときはふつうの生活をさせてOK。ふつうの赤ちゃんと同じように外遊びをさせても大丈夫です。また薄着や乾布摩擦で皮膚を鍛えるのも効果的です。

室内のホコリやダニ対策をしっかりと
　

気管支ぜんそくの原因は、その大半がハウスダストと呼ばれる家の中のホコリやダニであると考えられています。ですから、発作を予防する意味でも、部屋をきれいに掃除しておくことが大事。寝具は日に干したり、乾燥機などを使ってきちんと乾燥させ、その後掃除機でダニを吸い取るようにしましょう。カーペットはダニが繁殖しやすいので、なるべく使わないように。羽毛布団やそばがらの枕などもやめたほうがよいでしょう。そして部屋の通気をよくするため、晴れた日には窓を開けて、空気の入れかえをしてください。
　犬、猫、ハムスター、鳥など毛のあるペットは飼わないほうがよいでしょう。タバコの煙も気管支の過敏性を促すので避けてください。
